Powers of Attorney
A Power of Attorney (POA) is a legal document whereby an adult appoints one or more people to look after his/her legal and financial affairs.

Health Care Directives
Representation Agreements are documents that allow adults to appoint representatives to make their health and personal care decisions in the event that they are not able to. If you become incapacitated, your representative is the person(s) who you have authorized to make health care decisions and choose the appropriate place for you to live on your behalf.
The Representation Agreement only comes into effect if the individual who has made the Agreement becomes incapacitated.
